When I looked (~2 yrs ago) into what was involved in getting a credit card merchant account - even via worldpay - it was crazy. The terms were along the lines of "if there's a chargeback, you are completely on your own, unless you have carried out all security steps we advise you are reasonable [postcode/address check, cvv, 3d-secure] and you comply with their results, and even then if there is a chargeback we can decide not to help you out and you have no right of appeal".
Speaking as someone sitting at the sharp end of this mess I thought I'd comment. *ALL* Maestro cards are required to have 3D secure verification for transactions to succeed. So that's one card brand down (though it is effectively a debit card). Interestingly there will be a mastercard "debit" in the future though this is not to circumvent SecureCode but to allow rebilling of the card without Mastercard having to admit they cocked up there.
Ultimately the card schemes can insist on 3D secure across the board and have threatened (and imposed, I gather in the case of mastercard) large punitive fines for non-compliance. It seems almost a certainty that this will be universal, what I think of that should be clear enough.
I don't recall my lloyds credit card ever requesting 3D but that was a while ago.
Not IME: a couple of years ago, one particular site (joker.com) insisted on 3d-secure for an online purchase, and required one to jump through ludicrous hoops if one's bank didn't support it. (Fill in your details on this web form; print it out; sign; tape your physical card to it; fax, or scan and email, to their accounts people.)
